This task struck me as particularly amusing, since if you think about it, documentation is usually of documents which are in turn documenting some sort of happening. Where I work, it goes in many many levels. First, I get the initial documents.

Then I put the documents in the folder after assigning a number and documenting what the documents said.

After that I move to the clipboard, where I document what I documented on the folder plus some additional information from the original documents.

And then, just in case all the paper documents suddenly combust or disappear, I enter in all of the previously documented information into a database.

Actually that's more so there's an easily searchable way to figure out where the initial documents and subject thereof are currently located. And finally we have the documentation of the documenting of the documentation of the documents.
